Thank you for considering our practice as your Health Care Provider.

 

We are committed to the success of your medical treatment and care. Please understand that your payment for our medical services is part of this success.

 

 

We would like to share these thoughts with you:

 

Because of the constatly rising healthcare premiums, patients are changing insurance plans more frequently than ever before. In order to avoid delays or misunderstandings regarding your claim processing, we are asking you to provide us with a proof of your current insurance coverage, as well as your current home address and your phone number every time you make your appointment. This information is essential for processing your claims. Our office will submit claims to your insurance company on your behalf.

 

However, patients ultimately have the responsibility for their share of the bill. After all is said and done, your policy is a contract between you and your insurance carrier only.

 

We accept virtually all medical and health insurances. New patients are advised to confirm their current health insurance with our office at the time of the appointment inquiry.

 

We welcome self-pay patients, and we may offer special rates at the time of your visit.

 

Complete billing information and our payment policy will be provided at the time of your appointment. Patients not having a valid insurance or unable to provide adequate billing information are responsible for the total payment of their bills.

 

It is a requirement of your insurance company that you pay the co-payment at the time of the service. We accept cash, personal checks, Visa, Master Card and American Express.

 

We may ask you to reschedule your appointment if you do not have your co-pay available at the time of service, or if prior arrangements approved by our billing office are not secured.

 

**Most of our patients have some type of medical insurance. We will file claims on your behalf as a courtesy to you.  You will be asked to sign the Benefits Assignment Portion of the insurance claim form before the form is submitted. Signing this form allows the insurer to pay your benefits directly to our office.  Should you prefer to file for your insurance yourself, we will provide you with an itemized bill.
