The day of your appointment

 

 

Because of the premium costs, patients are changing insurance plans more frequently then ever before. Therefore, it is very important that we have your pertinent information on file. We are asking to provide us with your current insurance coverage, as well as your current home address and phone number every time you make your appointment.

 

If you are a new patient to our office, please arrive 30 minutes prior to your scheduled appointment and check in at the front desk. At your first visit, you will be asked to do the following:

 

·         Complete a new patient registration form

·         Complete a medical history form

·         Present you State issued identification card/Drivers’ license

·         Present you current Health Insurance card

·         Acknowledge receipt of our Notice of Privacy statement

·         Sign an authorization to release confidential health information

 

Please acknowledge that the “Red Flag Identity Theft Rule” recently implemented by law, require us to ask and confirm your Photo ID every time you visit doctors office. It becomes absolutely necessary, if you are a new patient.

 

If you are a returning patient, please arrive 10 minutes prior to your scheduled appointment. At your return visit our staff is required to provide you with a short registration form to confirm your name, address, telephone numbers and your current insurance information to update our records.

 

If your previous visit to our office was more than 90 days ago, we require that you complete a New Patient Registration form.

 

If there is a reason to cancel your already scheduled appointment, as a courtesy to other patients please call our office as soon as possible to re-schedule it. Patients seeking our medical advise that day would realy appreciate if we can accomodate them at the time they need or could be seen.

 

Well Child Visits, School Physicals and annual Appointments

 

If you have school-aged children, we encourage you to contact our office in the spring to schedule fall school and sport physicals; that way, we will be able to see your children in timely matter before the start of the school year.

 

Follow-up well child examinations should be scheduled before you leave the office to ensure we can see your child in the appropriate time frame. Please note that all patients under age of 18 must be accompanied by their legal guardian.

 

We encourage our adult patients to plan in advance for annual, routine physical examinations since they may take several weeks to schedule.
